Create a small object, make it transparent, and make it emit light.
For the simplest case, you can do the latter in the object editor; if
you want different lighting levels or colors, you'll need to do it
from a script in the object, with e.g. voice controls. Then attach
that object to something stable on your face, typically your nose or
chin. Then adjust its position (using "highlight transparent" so you
can see it) to get the lighting angle that works best. I find putting
it about a half a meter away, slightly above the head and offset a bit
from the centerline, makes for a nice effect.
